Name Change

Complete the Change of Name Form (1 p) and mail it to The Hartford at the address provided at the top of the form. Provide the proper supporting documentation (for marriage, divorce, adoption, etc.). The policyowner must sign with both his/her previous and current names.

Address Change

Complete the Change of Address Form (1 p) and mail it to The Hartford at the address provided at the top of the form.

Complete Change of Beneficiary Form (1 p) and mail it to The Hartford at the address listed at the top of the form.
